To Apply For Retirement/Separation
Benefits
- Accomplish CEAP Retirement Form No. 9 (Application
for Benefits) in triplicate.
- Indicate in the upper portion of the Application
Form to which benefit the applicant is entitled
to. The signatures of the applicant/member and
the school’s authorized signatory are required
and all pertinent information must be filled up
for the Application to be processed.
- The application for the payment of benefits
must be filed by the member with the CEAP Retirement
Plan Office at least 30 days in advance of the
date of actual retirement, resignation or separation.
- Benefit computation will be done by ASC Philippines,
Inc., the Fund’s backroom provider and check
preparation will be prepared by MetroBank Trust
Banking Group. The check will be made out to the
applicant/member and mailed to the School.
- The School shall have the first lien on the
benefit to be received by the applicant/member
to cover all his liabilities to his Participating
Employer and all losses, costs and expenses which
the School may incur through his dishonesty, defalcation,
theft or any other act.
