To Apply For Death Benefit
- Accomplish CEAP Retirement Form No. 11 (Application
for Death Benefit). This application for death
benefit will be filed by the School in behalf
of the beneficiaries of its deceased employee-member,
duly signed by the School’s authorized signatory.
- The following supporting documents must be
attached to the Application, as follows:
-- Xerox copy of certified true copy
of Death Certificate
-- Xerox copy of certified true copy of Marriage
Contract (If beneficiary is the surviving spouse).
-- Attending Physician’s Statement (CEAP
Retirement Form No. 11-A)
-- Xerox copy of certified true copy of Official
Police Investigation Report ( In lieu of Attending
Physician’s Statement in case of violent
or sudden death with no attending physician).
-- Affidavit of Guardianship of Minor ( In case
beneficiary is a minor).
-- Deceased employee’s latest monthly
salary
- Forward the application with the attached documents
to the CEAP Retirement Plan Office for processing.
- ASC Philippines Inc. will compute the death
benefit due the deceased employee’s beneficiary
and MetroBank Trust Banking Group will prepare
the check payable to the designated beneficiary.
It will then be mailed to the School where the
beneficiary can pick-up the check.
- Allow 1 to 2 weeks for the processing of the
Application.
